In-Home Services

In-Home Services are designed to enable older adults to remain in their own homes. Types of services include:

• Homemaker: Assistance with light household chores, such as laundry.
• Personal Care: Assistance with daily living activities such as bathing and dressing.
• Respite: Companionship and assistance in the absence of the primary caregiver

Case Coordination and Support

Trained nurses conduct client assessments and arrange essential services for age 60+ older adults who need minimal support or assistance. Please contact the office serving your county below:

Care Management

Support services are arranged by a skilled nurse and social worker to ensure that frail adults, at risk of nursing home placement, remain as independent as possible. Please contact the office serving your county below:

Care Plus (MI Choice Medicaid Waiver Program)
