Providing incremental levels of care based on the physical and/or financial needs of the client. Valley CHOICE service options include:


In-Home Services

In-Home Services are designed to enable older adults to remain in their own homes. Types of services include:

  • Homemaker: Assistance with light household chores, such as laundry.
  • Personal Care: Assistance with daily living activities such as bathing and dressing.
  • Respite: Companionship and assistance in the absence of the primary caregiver

Case Coordination and Support

Trained nurses conduct client assessments and arrange essential services for age 60+ older adults who need minimal support or assistance. Please contact the office serving your county below:

Care Management

Care Plus (MI Choice Medicaid Waiver Program)

Trained nurses and social workers conduct comprehensive client assessments for disabled persons age 18-64 and age 65 + elderly who meet nursing home eligibility requirements. Residents of Genesee, Lapeer and Shiawassee Counties please contact:
